On this page you will find the forms that need to be completed on your first visit and documents that provide you with important information about our services.  If you prefer to complete these forms ahead of time, please feel free to download them, fill them out at home, and bring them to your first appointment.  Otherwise, if you’d rather fill out the forms at our office, please plan to arrive at The Summit at least 20 minutes ahead of your first appointment time.

• Directions to The Summit

The Summit building was built in 2007, but some maps don’t keep up with progress!  You may find that directions to our address are not correct in some mapping sites online or in personal navigational systems.  This document has the correct directions to bring you directly to The Summit!

• Client Assistance Fund (CAF) Application Form

The Summit’s Client Assistance Fund (CAF) is available to help underwrite and supplement the fees of those clients who may not be able to afford our full standard fee.  If you think you would need to apply for our CAF support, please download and fill out this form.  Remember also to bring the supporting documentation that verifies your income to apply for these funds.  Click here to learn more about the Client Assistance Fund.
